Diagnosis

An issue came up and your priority is on getting in and resolving it.  You'll be seen by the highly trained outpatient intensivist working at the time of your choosing.  Your personal provider will be updated immediately.  

All new patients, use this appointment type for your first visit.  Established patients, use this appointment type for new issues as well as worsening problems with established issues.

Disease Management and Prevention

You already have a plan of care with your personal provider, and now it's time to follow-up on a disease or to do a yearly preventive exam.  You priority is on seeing your own provider since he or she knows you best.  If your provider isn't available immediately, that's OK, because these visits are less urgent and can be planned further out. 

This appointment type is not available for patients presenting for their first visit.
