BlueSky Health Achieves Patient Centered Medical Home Designation for 4th Straight Year

BlueSky Health in Houghton has been selected as a Patient-Centered Medical Home (PCMH) designated practice by Blue Cross Blue Shield of Michigan for the 2012 designation year. It It is the only health care practice in the Western U.P. region to be selected for each of the four years that the program has been in existence. 

Dr. Dovin Elects Not To Return From Maternity Leave: Opts To Be "100% Mom For Now"

Dr. Dovin has decided not to come back from
maternity leave in order to be, in her words, "100% Mom for now".
 
"We appreciate her dedication to family, and are
fully supportive of her decision," said clinic physician, Dr. Jeff Huotari.
Dr. Huotari had returned to the clinic full time to care
for Dr. Dovin's patients during her six month maternity leave. He expects a
"seamless transition" as he remains on full time to continue that care along
with nurse practitioner PeriAnn Wasie.
 
 

BlueSky Health Designated by BCBSM for 3rd Consecutive Year

(Houghton, MI) BlueSky Health has been selected as a Patient-Centered Medical Home (PCMH) designated practice by Blue Cross Blue Shield of Michigan for the 2011 designation year.  It is the only health care practice in the Western U.P. region to be selected for the designation for the third consecutive year. 

"We are excited to continue to lead the PCMH effort in this region," said BlueSky Health founder Dr. Jeffrey Huotari.  "We are also pleased that, from the first time, both local hospitals had several clinics join us in working for and receiving this designation.  It is our hope that with continued effort, each clinic and physician in our region can achieve the patient-centered changes necessary to receive this status."

Dr. Kimberly Dovin and Dr. Jeffrey Huotari, BlueSky Health's physicians, were also each individually selected as PCMH designated providers for the third consecutive year, which is the longest duration for any physicians in the region. 

According to Dr. Huotari, a patient-centered medical home is a medical clinic that has committed to changing the delivery of care to a proactive, coordinated experience that places a patient and their personal physician or care team in charge of all care delivered.  It leverages technology to improve efficiency, and access and quality are core features. 

BlueSky Health implemented the model in 2006.  Blue Cross Blue Shield of Michigan has endorsed the model as the desired method of primary care for its members since 2009 when the designation program began.

Shared Visits

Shared Visits!

Patients -- Good News!  BlueSky Health is looking to implement shared visits.  These visits will happen monthly and will be led by our providers.  The visits will be focused on various chronic diseases, living with a chronic disease, discussing barriers and overcoming those barriers, relating with other patients and receiving peer support, taking your vitals, and answering your questions related to your disease, prescriptions, and the like. 

We will be taking the first fifteen patients that register for these visits.  What qualifies you to register for a shared visit?  You must be a patient of BlueSky Health and have been seen by one of our providers within the past year (ranging from May 2010 to May 2011). Are you interested?  Call Leanne at (906) 483-2200 and mention "shared visit" in your message, email her at lnordstrom@blueskyhealth.org, and she will get back to you to enroll you into the shared visit. 

Cost:  We can bill your insurance for shared visits OR you can pay our self pay rate.  The cost to attend a shared visit will be $75.00.  We will collect all self pay payments and copays at the visit, please come prepared for us to do so.  Thank you.

A Warm Welcome


BlueSky Health would like to extend a warm welcome to Brenda Kinnunen! Brenda has joined BlueSky Health as a receptionist. Her involvement will be to welcome our patients, assist them with any need they may have, and work to keep the clinic operating in a clean, smooth, and comforting way.

 

She brings youth, a smile, and a courteous personality to the clinic. Brenda is currently attending Gogebic Community College to earn an Associate's Degree in buisness. She enjoys running, road trips, shopping, and hanging out with her family and friends.

A Model Practice :  Houghton Clinic chosen for Michigan Case Study

HOUGHTON - When Dr. Jeffrey Huotari designed the space for his new BlueSky Health clinic in 2006, he did it with a focus on the Patient Centered Medical Home philosphy on which he intended to base his new practice.

Because he was so successful with the PCMH concept in his practice, Huotari was chosen as one of the 13 practitioners in a case study done by a coalition of Michigan physicians organizations.  The study was released in January. 

Huotari said PCMH is based on several concepts, including:  everybody should have an ongoing relationship with a personal care provider; caregivers must be accessible, either the same day or next day; coordinated care with other health care professionals and organizations goes through the primary care provider; decision-making must be based on patient needs.
