Patient Centered Medical Home

In 2009 BlueSky Health was designated as a Patient-Centered Medical Home (PCMH) through the Blue Cross Blue Shield of Michigan Physician Group Incentive Program.  The principles of a Patient Centered Medical Home is clearly defined by The Patient-Centered Primary Care Collaborative, which solidifies the mission and purpose of the PCMH nationwide initiative. 

Joint Principles of PCMH

Personal physician - each patient has an ongoing relationship with a personal physician trained to provide first contact, continuous and comprehensive care.

Physician directed medical practice – the personal
physician leads a team of individuals at the practice level who collectively take responsibility for the ongoing care of patients. (Read more)
